My Kid’s Chart

My Kid’s Chart is our new Pediatric Patient Portal tool. Once you register for the Portal you will have access to a lot of information about your child.

Immunizations, school and camp forms, and more will be available for you to download at home at your convenience. Additional information may be accessed upon request.

There is a secure messaging feature which will allow you to send non-urgent messages to the office and doctors as well. Messages will be check frequently and in most cases replies will be given within 2 business days.

We value our patient’s privacy. Access to patient information is turned off on the 18th birthday. Of course, those patients may register for their own accounts as well.

In order to sign up for My Kid’s Chart, please download, complete, and return the My Kid’s Chart Registration form.

Once registered you can bookmark the url, save it to your smartphone, or use the Patient Portal button on the top of each page of our web site.

Expecting Parents

Congratulations on what is about to be a very exciting, wonderful, and rewarding part of your lives! We welcome the opportunity to share this experience with you, and to add our support, guidance and information along the way.

Expecting parents can call our office and schedule a free Prenatal visit. It is a great opportunity to meet one of our doctors, get a look at the office, and ask any questions about our practice, or your pregnancy. To schedule a prenatal visit, just contact the office by phone, or visit our contact us page.

If you have chosen us to be your Pediatricians (thank you!), and you are delivering at Valley hospital in Ridgewood, or Englewood hospital in Englewood, you simply need to let the delivery staff know that we’ll be your doctors, and they’ll alert us when the baby arrives. We’ll come to the hospital each morning to make sure things are okay and answer questions.

If you’re delivering at a hospital at which we don’t have privileges, schedule an appointment with us for 2-3 days after your discharge from the hospital.

Prospective patients can also contact our office for a “meet and greet”, so check out our office and meet a doctor.

If you’re planning your first visit with us, please fill out our Welcome Packet to get the necessary forms to fill out in order to make your first visit with us more efficient.

In addition, you should fill out a Record Release Form, which will help us receive any previous medical records you or your children may have have. Should you have any questions or problems with this process, please contact our office manager Marie at 201-666-1001.
